What to Expect During the Procedure

Implants can be placed either on top of the chest muscle or under it. Many people think implants beneath the muscle feel more natural. It can also keep the implant in place. Once your incision location has been specified, Dr. Aydin then creates a pocket in the tissue either under the pectoral muscle or directly behind the breast tissue on top of the pectoral muscle. Once placed, Dr. Aydin checks symmetry and balance before closing your incisions. Incision locations can be around the areola, in the breast crease on the bottom of the breast, or in the armpit. Dr. Aydin will decide which incision is best for your case.

Silicone vs. Saline

Saline breast implants are silicone shells filled with sterile salt water. They are inserted empty and filled once in place. A smaller incision is required, and in the case of a rupture, the body naturally absorbs the salt water.

Silicone implants consist of a silicone shell filled with medical silicone gel. They are inserted already filled. The general consensus is that silicone implants feel closer to natural breast tissue than saline implants. More recently, “gummy bear” implants have been added to the mix. Their consistency is even more firm, and the silicone doesn’t leave the shell if there is a leak. This is the disadvantage of silicone implants: if there is a rupture, the silicone can leak out of the shell. For this reason, regular mammograms are required with silicone implants to ensure they aren’t leaking.

Choosing Your Implant Size and Shape

With augmentation, you will also work with Dr. Aydin to choose your implant size and shape. Size is measured in cubic centimeters (cc’s). The two main shapes are round or teardrop. Teardrops are thought to be more like the natural breast shape, and they are less prone to rotation once in place. There are also different degrees of projection, depending on your preference.

Texture

Implants are made with either a smooth or a textured surface. In general, smooth implants cost less and are more common. Textured implants were initially conceived to reduce the chances of capsular contracture (the constriction of scar tissue around the implant), although there is no valid claim to this. The textured type is more common with the teardrop shape, as the rough texture adheres to tissues, helping the implant stay in place.

Shape

The standard shape of a breast implant can be rounded or tear-shaped. Each shape serves a different aesthetic function as it pertains to the patient’s goals. Rounded implants produce a shapelier curve on the top portion of the breast. Teardrop implants are shaped to copy the natural appearance of the breast so they will not appear as high.

Profile

Breast implants come in a variety of different profiles. The selection is based on how enhanced the breasts’ projection would be, as well as how wide the implant will be on the chest wall in relation to the base width. Since the silicone implants are already prefilled, the standard profiles come in low, moderate, high, and extra high. Saline implants, however, come in variants that are either prefilled or the silicone shell is inserted and then filled with saline solution up to the patient’s preferred profile. Choosing the right profile can sometimes be difficult, and it is up to a skilled surgeon to help guide the patient accordingly. High-profile breast implants will have greater projection but less width on each side, whereas a moderate profile, for example, will not have as much projection but will be broader.
